Preheat your oven to 375°F (190°C).
In a large bowl, mix together ground chicken, bread crumbs, milk, egg, garlic powder, onion powder, salt, and pepper.
Press half of the meat mixture into the bottom of a loaf pan.
Layer the ham and cheese slices over the meat, leaving a small border around the edges.
Cover with the remaining meat mixture, pressing the edges to seal.
Bake in the preheated oven for 45 minutes, or until the meatloaf is cooked through.
Let stand for 10 minutes before slicing.
